Although Integrity Family Care is closing, patients will continue to receive access to care and to their records. Understand that our patients need not make any changes and they will continue to have the same access to care and to their records. In fact, we ask that you NOT rush to transfer your records as we are doing everything we can to ensure a smooth transition.
We have reached out to patients with upcoming appointments over the coming weeks to ensure that they have adequate medication refills.

If you choose to stay with the Huntsville Hospital Physician Network, you need NOT transfer your records. If you wish to use a different provider, please wait a couple weeks before requesting your records to ensure that the response will be timely.
