2. To save time at your first appointment, print and fill out the patient forms and bring them with you.
3. If you’d like to pre-register for our patient portal, you may do so. Otherwise, you will receive a link with instructions after your first appointment. If you don’t, please let us know! Read more about our patient portal.
4. Arrive 30 minutes prior to your first scheduled appointment in order to process new patient forms, check insurance coverage, and answer any questions you may have. Co-pays are due at the time of your appointment.
Before You Arrive
We strongly recommend you contact your insurance company before your visit to find out if they require you to select a Primary Care Physician (PCP) prior to appointment. Many plans require this step. If the answer is YES, you will need to set your PCP to one of the providers in our office prior to your appointment.
We recommend you contact your insurance company prior to your first lab appointment. Your plan may require you to use a preferred lab. If so, make sure you notify the lab when they draw your blood to avoid any unexpected bills.
Make sure to bring your a photo ID, all insurance cards, any medications, and medical records you have. If you have printed and filled out your new patient forms, don't forget to bring those with you!
